The community liaison team

The Neonatal Community Liaison Team comprises of 3 experienced Neonatal Sisters; they offer support, advice and teaching to prepare families for home. The team also provide a community service 6 days a week. This means that families can be given additional support in their own homes after discharge from hospital. Support is particularly offered to families with babies who:

  • need additional oxygen,
  • need some feeds to be given by naso-gastric tube,
  • have a discharge weight less than 2.1 kilograms ( 4lbs 10ozs),
  • weigh between 2.1 (4lbs 10ozs) and 2.5 kilograms (5lbs 8ozs) on discharge and who still
  • need some support with feeding and managing their temperature,
  • have additional special needs.

Advice and support from the community liaison team is available to any family who feels they need it. If you would like some extra support in the form of teaching, a home visit or simply a phone call once you are at home then please do not hesitate to talk to any of the team - this is a service open to all parents.
